Region of Halton

In the Region of Halton, service providers from three agencies (Community Living North Halton, Community Living Burlington, and Halton Region Children’s Developmental Services) have come together to implement the F-words into practice. All of these agencies provide developmental services to children (birth to 12-years-old) and their families in Halton Region using a family centered model. Below are some examples of how the F-words are being used in Halton region (e.g., incorporated into documents/reports, developed educational materials, etc.).

Family Plan & Family Plan Leading Questions:

Child & Family Profile:

The Child and Family Profile Document is used in the first 1-3 visits by a service provider with a child and/or their parents/caregivers. It is used to capture information and initial observations obtained in the home or in a community setting. The document has been revised to include a section to prompt/ask parents/caregivers to talk about their child using the F-Words. 

F-Words Family Handout:

This new handout provides information about the F-words to families and caregivers. Additionally, this handout can be used to share information about the F-Words with other interested parties, such as community partners.

F-WORDS PROFILE:

This F-words Profile is based on the adapted version by Children's Treatment Network. The Profile is used to record information (e.g., strengths, interests) that the family/caregivers share about their child using the F-Words framework. The profile encourages staff and families to engage in discussions about the child using the F-words at various points throughout service. Complementary guidelines were also developed to support service providers use the F-words Profile.

VISIT SUMMARY:

The Visit Summary Document is used to provide information, strategies, or notes to the family, center, school or other service providers involved with the family. A blurb on the F-words was embedded in the form to prompt the use of this language in sessions between service providers and parents/caregivers.
